CLIA Laboratory Compliance and Personnel Requirements Explained
A breakdown of CLIA test complexity categories, personnel qualification rules, proficiency testing, and certificate types for clinical labs.
Almost every clinical laboratory operating on US soil — from a single point-of-care glucose meter in a physician's office to a full-service hospital reference lab — falls under the Clinical Laboratory Improvement Amendments (CLIA). CLIA doesn't regulate diagnoses or treatment; it regulates the people, processes, and quality systems behind the test result a clinician relies on. For compliance officers and lab managers, the practical question is usually narrower than "what is CLIA" — it's "what complexity category is this test, and does the person running it actually qualify to run it?" This guide answers both.
How CLIA Categorizes Tests
CLIA sorts every laboratory test into one of three complexity categories, and the category drives almost everything else — which personnel can perform the test, how much oversight is required, and whether proficiency testing applies.
- Waived tests are simple tests with a low risk of an incorrect result if performed as the manufacturer intends — think rapid strep tests, urine dipsticks, and many home-use or point-of-care devices. A lab running only waived tests needs a Certificate of Waiver but is not subject to routine CLIA inspections or the full quality system requirements that apply to nonwaived testing.
- Moderate complexity tests require more training, judgment, and instrument maintenance. A subcategory, provider-performed microscopy (PPM), covers specific microscopy procedures performed personally by a physician, midlevel practitioner, or dentist during a patient visit.
- High complexity tests demand the most training, experience, and judgment, and include any laboratory-developed test or any FDA-cleared test that a lab has modified from the manufacturer's instructions — modification automatically bumps a test into the high complexity category regardless of how simple the original test was.
The FDA scores commercially available tests against seven criteria — covering the knowledge, training, and judgment needed to operate the test, and the complexity of reagent preparation, operational steps, calibration, quality control, troubleshooting, and result interpretation — to decide whether a test lands in the moderate or high category.
CLIA Certificate Types
A laboratory's CLIA certificate defines what it's legally permitted to test:

Personnel Requirements by Complexity Category
Personnel rules only kick in for moderate and high complexity testing — waived testing has no CLIA-mandated qualification requirements, though manufacturer instructions and good practice still apply. In December 2024, CMS finalized its first major update to CLIA personnel requirements since 1992, and the changes are worth knowing because they reshape who can hold key laboratory roles:
- Laboratory director: a physician director must now hold board certification in anatomic or clinical pathology — board eligibility is no longer sufficient on its own. Non-physician directors can qualify with a doctoral, master's, or bachelor's degree in a qualifying scientific field, combined with relevant laboratory experience and continuing education.
- Technical consultant / technical supervisor: the update created a new pathway allowing an associate degree holder with four years of qualifying laboratory experience to serve as a technical consultant for moderate complexity testing, alongside a permanent qualification pathway for military-trained laboratory specialists.
- Testing personnel: nurses holding a BSN can now perform high complexity testing provided they complete additional laboratory science training; military-trained laboratory technicians permanently qualify for moderate and high complexity testing; and standalone physical science degrees were removed as a qualifying pathway. Blood gas analysis keeps a stricter, specialty-specific requirement tied to a respiratory therapy or cardiovascular technology background.
Personnel already employed in a qualifying role as of December 28, 2024 are generally grandfathered under the prior rules, so training programs need to clearly document who is covered by grandfathering versus who must meet the new pathways.
Competency Assessment
Meeting the initial qualification bar is only the starting point. CLIA requires ongoing competency assessment of testing personnel performing moderate or high complexity testing: at least twice during an employee's first year (roughly every six months), and at least annually after that. A compliant competency assessment covers six specific elements — direct observation of routine testing, monitoring of result recording and reporting, review of QC and maintenance records, observation of instrument performance checks, evaluation using previously tested or blind samples, and assessment of problem-solving skills — and all six need to be documented, not just performed.
Proficiency Testing
For regulated analytes, labs performing nonwaived testing must enroll in a CMS-approved proficiency testing (PT) program. These programs send unknown-value samples three times a year, with five "challenges" per shipment for each regulated test. A lab's performance on these samples is one of the clearest external checks CMS has on whether day-to-day testing is actually reliable — and referring a PT sample out to another laboratory for testing, rather than testing it in-house as required, is treated seriously enough that it can trigger automatic certificate revocation for at least a year.
